Braces emergencies: what to do tonight, and what can wait
A poking wire at 11pm, a bracket off before a wedding, a lost aligner. What to do at home right now, and which ones actually need an early visit.
Almost every orthodontic emergency is uncomfortable rather than dangerous, and almost all of them can be made bearable at home with things you already have. The genuinely useful skill is knowing which is which, so you are not driving somewhere at 10pm for something that a piece of wax would have fixed.
Keep these five things at home
- Orthodontic wax. We give you some at fitting. Ask for more before you run out, it solves more problems than anything else here.
- A clean nail clipper or small wire cutter, kept aside for this purpose.
- A pencil with an unused eraser. The best tool for pushing a wire out of the way.
- Tweezers.
- Salt, for warm rinses.
A wire poking your cheek
The most common one by far, and it usually starts a few weeks after an adjustment as teeth move and the wire end emerges behind the last bracket.
Do this: dry the area with a tissue, take a small piece of wax, roll it into a ball, and press it firmly over the wire end. If the wire is long enough to bend, use the eraser end of a pencil to push it flat against the tooth. If it is genuinely too long and wax will not stay, a clean nail clipper can trim the very end, with a folded tissue behind it so the cut piece is not swallowed.
Then: message us. This does not usually need an emergency visit, but tell us so we can either see you or plan to fix it properly at the next appointment.
A bracket has come off
Usually from biting something hard, and usually it stays attached to the wire rather than falling out.
If it's still on the wire: leave it there. Cover it with wax if it is rubbing. Do not try to reposition it, and do not pull it off, because it is easier for us to re-bond a bracket that is still in place.
If it has come out completely: keep it, bring it with you. Wax over the exposed wire end if it is uncomfortable.
Urgency: low. A single loose bracket in the middle of the arch can usually wait days without affecting treatment. A loose bracket on a front tooth, or several at once, is worth an earlier visit. Message us and we will tell you honestly which yours is.
A band has come loose
The metal ring around a back molar. If it is completely off, keep it and call us. If it is loose but still in place, leave it and avoid chewing on that side, but do get seen reasonably soon: a loose band traps food underneath and that is a genuine decay risk rather than just a nuisance.
General soreness after an adjustment
Expected, normal, and worst in the first 48 to 72 hours after fitting or an adjustment. It feels like pressure or a dull ache rather than sharp pain.
What helps: ordinary pain relief taken at the first sign rather than once it has built up. Cold water or something cold to eat. Soft food for two days. A warm salt water rinse for sore gum spots. Chewing something soft, oddly, often helps more than resting the jaw, because gentle movement increases blood flow.
When it isn't normal: pain that is worsening after day three rather than settling, pain focused on one specific tooth, or pain with swelling. That needs a look.
Aligner problems
- Lost an aligner. Go back to the previous set immediately and message us, do not skip forward to the next one. Skipping ahead is the most common way aligner treatment goes off track.
- Cracked an aligner. Keep wearing it if it still seats. If it does not, go back to the previous set and tell us.
- It won't seat fully. Use your chewies for 10 to 15 minutes. If it still will not seat after a day, go back a stage and message us rather than forcing it.
- An attachment came off. Keep wearing the aligners, tell us, and it will be re-bonded at your next visit. It is rarely urgent on its own.
A retainer problem
This one is more time-sensitive than people assume. A lost or broken removable retainer needs replacing quickly, because teeth begin drifting within days, not months. If your retainer breaks, message us the same week, and in the meantime wear it if it still fits at all.
A fixed retainer wire that has come unstuck at one end should be seen soon too. The loose end can irritate the tongue, and the teeth it was holding will start to move.
Things that are genuinely urgent
- Facial swelling, especially with fever or difficulty swallowing. This is an infection, not an orthodontic problem, and it needs same-day attention.
- Trauma to the mouth. A fall or a blow that has loosened, moved, or knocked out a tooth. Time matters enormously here.
- A wire embedded in the cheek, lip, or gum that you genuinely cannot free with the techniques above. Do not keep pulling at it.
- Bleeding that will not stop after firm pressure for ten minutes.
How to avoid most of this
The great majority of bracket breakages come from a small list of foods: hard nuts, ice, hard boiled sweets, popcorn kernels, raw carrots and apples bitten into with the front teeth, and chewy sticky things. Cutting hard food into pieces and chewing with the back teeth prevents most emergencies before they happen.
The other big one: wear the wax. Patients who use it in the first fortnight get far fewer ulcers, and the ulcers are what makes early braces genuinely miserable.
Common questions
Can I cut a poking wire myself?+
For the very end of a wire that has emerged behind the last bracket, yes, carefully, with a clean nail clipper and a folded tissue behind it so the piece is not swallowed. Never cut a wire anywhere else in the arch, and never cut a wire that is part of an active spring or loop. If in doubt, use wax and message us.
How quickly can I be seen for a broken bracket?+

Will a broken bracket delay my treatment?+
One bracket left for a few days generally will not. Repeated breakages will, because each one means that tooth stops being actively moved until it is re-bonded. If breakages keep happening, it is worth reviewing what is being eaten rather than accepting a longer treatment.
I have a wedding next week and a bracket just came off. What now?+
Tell us, and say why it is urgent. Front-tooth aesthetics before an event is a legitimate reason to be seen early and we would rather know than have you turn up on the day disappointed.
